<p>Utilizing reduced depth of penetration during intercourse may reduce sex-related pain dramatically or barely at all, depending on the individual, but while other treatments are being considered or pursued, it is one manner in which sexual activities can be maintained while minimizing dyspareunia-related pain.</p>

<p>An example of a position that results in maximum (deepest) penetration is when a woman lies on her back (in the missionary position) with her legs pulled up to her torso.  In this position, a male partner will be penetrating as deeply as possible (and therefore making contact with the most tissue on the inside of the vagina).  In some cases, this may result in maximized dyspareunia-related pain.</p>
<p>An example of a position that would result in reduced or minimal penetration would be a woman still lying on her back, but with the man&#8217;s legs straddling hers on the outside (her legs are therefore quite close together).  In this position, a greater proportion of the penis will remain outside the vagina and less inner vaginal tissue will experience intercourse-related contact.  In some cases, this is an effective way of managing dyspareunia.</p>

<p>Especially since dyspareunia and sexual issues can be difficult to talk about, even between romantic partners, one partner may not know or understand precisely why the sexual relationship is the way it is. <p>One typical pattern in men involved pain in the genitalia shortly after ejaculation.  This may be caused by infections in areas such as the bladder, prostate or seminal vesicles.</p>
<p>Pain may also be experienced at the moment of ejaculation, rather than immediately after, and this may indicate different physical causes of the pain.  Interstitial cystitis may be a cause of this pain concurrent with ejaculation.  Gonorrhea can also produce this symptom.</p>
<p>Persistent pain in the genitals during or immediately after ejaculation can be characterized as postejaculatory pain syndrome. <p>Since this publication, there has been a significant amount of promising research and practical application of the technique.</p>
<p>Shirin Ghazizadeh MD and Masoomeh Nikzad MD outlined one such study in <u><i>Botulinum Toxin in the Treatment of Refractory Vaginismus</u></i>, found in <i>Obstetrics and Gynecology</i> 104, 922-925.  The results in the sample size were quite positive with respect to the efficacy of Botox treatment (in this case, the sub-type Dysport).</p>
<p>Furthermore, Botox was found to be effective when tested against a placebo (salt water believed by the patient to be Botox) in a 2000 study by Shafik A.</p>

- Fear of painful sex<br />

<p>Medical and psychological professionals will often be particularly attentive to the psychological aspects and possible causes of the condition when <u>primary vaginismus</u> is involved (the patient has never experienced vaginal penetration without pain).</p>
<p>It can also be important to examine some psychological conditions that may co-exist with, be correlated with, or made more severe by vaginismus.  These can include depression and self-esteem issues.</p>
<p>A <u>physiological or physical approach</u> to treatment will focus specifically on the physical muscular spasms that are creating the pain.</p>
